Functional Medicine of Nevada is here to help you. Whether you need assistance finding a provider, have questions about your bill, or need to get in contact with your provider, it is our intent to efficiently connect you with the appropriate person who can promptly answer your questions.

Please do not send direct emails containing medical information. Emails that are NOT sent through the Patient Portal are not considered HIPAA compliant. We will respond to all emails with a phone call.

Please do not contact providers or team members via text message or social media messaging. They will not be able to respond on those platforms.

Patient Portal

We have an established a Patient Portal that you may use to communicate with FMN:

  • General communication
  • Questions and clarifications regarding clinical/nursing questions, new patient inquiries, supplement requests, billing inquiries, and scheduling/patient care requests
  • Straightforward, non-emergency, and non-urgent questions to your provider

The Secure Messages Center within the Patient Portal provides a secure, encrypted alternative to email for electronic communications with your medical provider. Your secure messages can be read only by select Functional Medicine of Nevada staff members.

**Prescription Refill: If you need a prescription refill, please call your pharmacy and have them fax us a prescription refill request form. Fax number: (775) 206-0002

How to send a secure message in your Patient Portal

  1. Visit
  2. Click the Secure Messages icon on the right hand side of the screen, and click “Send a Secure Message”.
  3. Fill out the form.
  4. Select who you would like to send the message to. 
  5. Click “Send Message”.
