Do I have to be in-person for my first visit?

We are happy to offer telemedicine options for most appointment types. Inquire with our patient care team about these offerings when scheduling your initial appointment.

How should I prepare for my first visit?

Before your first appointment, please fill out all of the intake forms and questionnaires listed on your patient portal profile. Login here.

Please bring your insurance card and driver’s license (or some other form of identification) to your initial appointment.

What should I expect during my first visit?

During your initial appointment, your provider will hear your story and review your health profile with you to gather information. Your provider will spend time listening to in-depth health history, past health issues, and ongoing health concerns. This visit will set the foundation for your journey with FMN. 

If you choose Functional Medicine of Nevada for primary care, one of the next steps will be to schedule your wellness (yearly physical) visit.

Most insurances that are not listed above are considered out-of-network, including: Medicare, Medicaid, etc. 

What are my options if FMN is out of network or if I don't have insurance?

For out-of-network patients, we offer a 15% cash discount for appointments. 

We can provide you with a detailed bill for most services that contain ICD-10 diagnostic codes and CPT codes for reimbursement. 

We accept all HSA and FSA cards as a form of payment.

Can FMN be my primary care provider?

Adult patients who are in-network are able to select Functional Medicine of Nevada as their primary care provider after they establish themselves as a patient at our clinic. After their initial functional consult, they can then be seen for annual wellness exams and acute visits.

What is FMN’s cancellation policy?

To avoid our no-show fees, please cancel an appointment at least 48 business hours prior (ex. before 10am on Thursday to cancel an appointment scheduled for 10am on Monday). 

The no-show fees for cancelling less than 48 business hours before or not showing up for an appointment are: $100 for physicals and new patient appointments and $50 for follow ups and nutrition appointments.

What is FMN’s policy if a patient shows up late for an appointment?

Our office policy is to reschedule patients who are more than 15 minutes late to an appointment. Although we understand that situations can arise last minute that can cause our patients to be late, we also want to provide the best appointment possible with the appropriate amount of time to cover everything. We’ve found that starting appointments more than 15 minutes late makes our providers late for their following appointments. In these cases, we’re more than happy to reschedule.

How do I set up the patient portal for my entire family?

A separate registration form must be completed for each family member. This creates a patient chart unique to that individual.
